Family Health Centers of Southwest Florida, Inc.'s medical
offices accepts assignment from Medicare, Medicaid and private
insurance. FHC provides assistance to its patients in
determining their eligibility to receive healthcare benefits
under Florida's rapidly changing Medicaid system. Family Health
Centers works with those who are uninsured or who have limited
coverage, and we offer a discounted fee program for those that
qualify based on family size and income at the patient's
request. Dental care is also provided.

The mission of
the Brain Injury Association is to improve the quality of life for persons with
brain injury and their families by creating a better future through
brain injury prevention, research, education, support services, and
advocacy. Assists individuals with brain injuries, their families
and professionals by providing: Statewide Information & Resource Center;
Support Services; Awareness & Prevention Programs; Legislative Advocacy;
& Education & Training. All resources are provided free of charge to persons
with brain injuries and their families who request information. Anyone
who might benefit from our support services, educational materials, or
other resource information is encouraged to participate in our
organization. Serves Charlotte, Collier, Glades, Hendry, Lee,
Manatee, & Sarasota Counties.

CARD serves children and adults of all levels of
intellectual functioning who have autism spectrum disorder
(including autism, Asperger's disorder, childhood disintegrative
disorder, Rett's disorder, or pervasive developmental disorder not
otherwise specified); an autistic-like disability (documented on an
evaluation that describes the characteristics that are being
considered "autistic-like" by the evaluator); a dual sensory
impairment; or a sensory impairment with other disabilities. It is a
requirement of the legislation that created CARD that we have
documentation of a qualifying diagnosis in order for an individual
to receive our services. For children aged 0 to 5 years, we accept a
developmental disability diagnosis with documented evidence of
autistic-like behaviors.
